JOB SUMMARY

The Palliative Care Clinical Coach reports to the Home and Community Care Manager and works under the direction of the Six Nations Wellbeing Department. The role drives practice change and improves access and quality of care across community organizations. The position collaborates with Six Nations community palliative care, Ontario Health, the Ontario Palliative Care Network, and related frameworks.

KEY DUTIES & RESPONSIBILITIES

  • Provide palliative care coaching, mentoring, and clinical guidance to staff and clinicians to strengthen primary-level palliative care competencies.
