Job Description

JOB SUMMARY: Supports Director of Admissions and facility leadership to assure admission processes are followed and complete and patient transitions into facility are satisfactory.
Pay Rate: $18.00-$26.50

QUALIFICATIONS:
Team member has to have High school diploma or equivalency.

Successful completion of Outmatch Assessment showing compatibility with role.

Organizational skills and communication skills a must. 

Has to be proactive and anticipate the needs of the patients and families.

Needs to take initiative and have excellent communications skills.

Must be pleasant, cheerful, tactful, and courteous to residents, relatives, visitors, and other personnel.
PERFORMANCE REQUIREMENTS:
Support director of admissions with assuring all data required for admissions is documented in EMR (including but not limited to sex offender checks, PASRR/HENS)
