Job Description
FeldCare Connects is a leading referral therapy company. We partner with home health agencies and have a network of therapist that we connect to the agencies. We are seeking a detail-oriented Admissions Coordinator to join our team. The Admissions Coordinator is responsible for efficiently and accurately entering patient referral information into the Electronic Medical Records (EMR) system. This role ensures that all referral data is correctly recorded, maintained, and processed, facilitating smooth patient transition from our application to the EMR system so that therapy services can be initiated.
Key Responsibilities:
Accurately enter patient referral information into the Electronic Medical Records (EMR) system, including patient details, referral sources, insurance type, and any specific information needed by the clinician.
Verify the completeness and accuracy of the patient referral and History and Physical exam before entry.
