Overview


Helio Health's In Community Program (formerly Circare) is designed to help individuals and families, who have encountered certain obstacles develop the resources and supports to live a satisfying, naturally independent life.
TheAdult Health Home Care Coordinator works with and oversees the process with individuals to develop and maintain a comprehensive plan for accessing and using services and supports necessary for a person’s health.
Pay Rate:$22.12 per hour
In addition to Helio Health’s comprehensive benefits package, this position is eligible for a $1,500 Sign-On Incentive!


Responsibilities


+ Conduct outreach, and assessment for Health Home services for individuals who are Medicaid Eligible and/or have active Medicaid.

+ Conduct eligibility screenings and Health Home enrollment.
