Intermediate Tier service is a multidisciplinary integrated health and social care service. We support people to live as independently as possible in the place they call home. We provide support for people at times of crisis, additional intermediate need or following a stay in hospital. We also deliver technology enabled care and provide equipment and adaptations for people in the place they call home. It’s a short-term service focused on promoting recovery, preventing unnecessary hospital admissions, and reducing reliance on long-term care facilities. This type of care supports people to regain their independence and return to their previous level of functioning.
