Job Overview

Work with our growing Advanced Clinical Practitioner team within the Rapid Response Team to provide assessment and interventions to prevent hospital admissions, facilitate discharges meeting the 2‑hour Urgent Community Response standard, and support patients on the Frailty and General Virtual Wards.

Main duties of the job

We're recruiting an ACP to deliver NHS England's Community Agenda. You will support people in their usual place of residence during a crisis, treating them at home when safe to reduce unnecessary hospital admissions.

As an Advanced Practitioner you will bring experience of working collaboratively within a community or acute setting, exceptional assessment, intervention and communication skills.
