Advanced Nurse Practitioner

Do you have excellent communication skills, an ability to work in partnership with

others, a patient focused approach and a desire to improve services to meet patient

needs? Then come and join our community dietetic team in Aberdeen City.

This postholder will work with frail patients (or those at higher risk of frailty) in the

community, providing group or one to one education sessions to support people to

improve their diet and health.

There is an exciting opportunity to be involved in a project to help identify people at risk of falls and frailty within general practice or within local community settings, and to develop and deliver suitable education programmes to meet their needs.

This post will be supported by a Health Care Support Worker and dietitians already working in this area.

We are actively engaged in the use of technology to deliver care wherever possible and there will be opportunities to develop this further.
