Advanced Practice Nurse - Primary Care Women's Health

Summary Central Ohio VA Health Care System is seeking a Nurse Practitioner for Primary Care. Duty location is in Columbus, OH. The NP provides advanced clinical practice, consultation, and management. Specifically for the Women's Health PACT teams. Will float to provide coverage for any Women's Health PACT team. This could include female-only panels as well as panels with a mixture of male and female Veterans. Responsibilities Major duties and responsibilities of the position: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members. Orders and interprets labs, tests, and diagnostic studies.
