About the Role
The primary objective of the C No More Project Care coordinator position is to pilot a 12 month mobile outreach care coordination service for people in Community Corrections or otherwise involved in the criminal justice system.
People on community corrections orders share similar risk factors for poor health as the prison population but may not access primary health care in the community. This population has not previously been targeted for primary health care in Victoria.
