Job Overview
We are looking to recruit two proactive and compassionate Assertive Outreach Support Workers to join our Community Mental Health Hubs (CMHH). One role will be based across Queen’s Park & Paddington hub (Woodfield Road) , and one within the South Hub (190 Vauxhall Bridge Road) .
We will support individuals with complex mental health needs who are at risk of disengagement, providing practical, person-centred care to promote engagement, independence and recovery. This is a rewarding opportunity to work as part of a supportive multidisciplinary team, with access to regular supervision, development opportunities, and the chance to contribute to an innovative and evolving service.
