About The Role

• Refine and develop integrated care models with a focus on transitional, pre-, and post-hospital workflows, to improve patient outcomes.
• Manage and analyse data to support decision-making.
• Operationalise new or developing models of health and care that integrate hospital and community.
• Perform data collection through administering surveys and data collation for department programmes.
• Engage and collaborate with healthcare partners in the community.
• Co-supervise and support the daily scheduling, charging and billing operations of the Community team.
• Additional duties as assigned by supervisor.

What We Are Looking For

• Bachelor's degree in Healthcare Administration, Business Administration, or related field
• Minimum 2 years of experience working in large organisations, particularly in project management or healthcare operations.
