Autism and Intellectual Disability Keyworker
The post holder will serve as a DSR System Navigators for children, young people, and adults (up to the age of 25) with autism and/or intellectual disabilities (ID) who have complex needs across North West London and are on the Dynamic Support Register (DSR)The post holder will hold a caseload of CYP/Adults and their families and care givers to support them to build resilience and navigate the local system
To work with key stakeholders including people with lived experience to identify gaps, inequalities and variation in the system to shape and model the service.
Main duties of the job
Delivery of the System Navigating function for children and young people (CYP) and adults with autism and/or Intellectual Disabilities (ID) who are on the Dynamic Support Register in the 8 NWL boroughs (Harrow, Brent, Ealing, Hillingdon, Westminster, Hounslow, Hammersmith and Fulham or Kensington & Chelsea)
