Community neuro caseload across home and community settings: Stroke (ESD), ABI, MS, Parkinson’s, MND .

What you’ll do

• Functional, goal‑focused neuro rehab in the home
• Cognitive/perceptual assessment, upper‑limb rehab, fatigue/tone management
• Reablement, equipment/adaptations, seating/positioning
• Home visits, risk assessments, ESD and safe discharge planning
• MDT working and clear documentation (e.g., COPM, GAS, MoCA, ARAT)

What we’re looking for

• HCPC‑registered OT
• Recent Band 6 community neuro experience (NHS preferred)
• Confident with domiciliary assessments and risk management
• UK RTW + enhanced DBS
• Car driver
