Behavioral Health Navigator I - UPMC Altoona

We are hiring a full-time Behavioral Health Navigator I to support UPMC Altoona!

The Behavioral Health (BH) Navigator I is responsible for the assessment and navigation of patients and families in collaboration with attending psychiatrists and other staff within the Emergency Department at UPMC Altoona. The BH Navigator I will develop initial treatment plans to facilitate disposition planning and assists with bed placement or disposition planning.

Shifts will rotate between days, evenings, nights as well as working rotating weekends. The incumbent will work 2 8-hour and 2 12-hour shifts. Must work some holidays.
