Job Description

Position Details
This position will appeal to OTs seeking a diverse community-based practice serving clients across the rehabilitation continuum. Duties include:
Hospital discharge planning and assessments

Cognitive assessments & rehab to increase client independence

Graduated activity plans to increase home/community functioning

Development & implementation of community integration plans

Case management to facilitate client access to other health services

Development and supervision of rehab assistant plans

Liaising with funders, family and healthcare team
Qualifications
Member of COTBC or eligible for registration

Driver’s license and access to a reliable vehicle

Excellent verbal and written communication skills

Aptitude for negotiation & consensus building
