 Practice Settings: Outpatient  
 Patients Per Shift: 15+  
 Support Staff: 5 RNs, 9 MAs, 4 APPs  
 Trauma level 2  
Requirements  
 Board Certified  
 Licensed in WA or IMLC  
 Fellowship - Preferred  
 ACLS, BLS  
 Provider Min # Shifts Required: 4 weeks per month  
 DEA
