Job Description
To provide basic care coordination, discharge planning, monitor/evaluate and education for clients and their families on Day Care Service received as well as Administrative support to Day Care Utilisation and Learning and Development. 

You will need to:

Information and Referral (I & R)
Manage referral sources, including emails, phone calls, and walk-ins, by evaluating client needs and matching them with appropriate services.

Coordinate the referral process with clients and external stakeholders, ensuring successful service referrals and optimizing lead conversions.
Service Coordination
Maintain and manage the administrative documentation of the cohort of clients who are categorized to require regular, maintenance care.
