Care Manager - Rockhampton

Mercy Community is seeking a strong clinical leader to join Leinster Place, our 80-bed residential aged care home in Rockhampton.

This is a key leadership role supporting the Facility Manager and providing clinical oversight, team guidance and accountability for resident care outcomes.

If you are an experienced Registered Nurse who understands aged care reform, AN-ACC, compliance and real clinical leadership.

About the Role

The Care Manager supports the Facility Manager in the overall accountability for clinical governance, quality and resident outcomes across the home.

You will:

• Provide hands-on clinical leadership and oversight
• Ensure care plans, assessments and documentation are compliant and meaningful
• Oversee risk areas including wounds, falls, medication, nutrition, hydration and continence
