Resthaven are looking for a Care Partner to provide comprehensive coordination and individualized case management and planning in partnership with Support at Home individuals.
Our Limestone Coast Community Services team have an opportunity for a Care Partner - Support At Home to join the team in a Permanent Full-time role. This role is based out of our Mount Gambier office but there is flexibility to base out of Naracoorte for the right candidate.
The Care Partner will work with the intake team to onboard individuals to the appropriate Support at Home, ensuring that approved services are provided as outlined within their Support Plan. Care Management and coordination are primary functions of the Care Partner role and will be optimized by working with the individual, their supporter, as well as the staff providing care and services.
