About the role
Churches of Christ are currently looking for Care Partner within our dynamic Home Care Darling Downs, as a Care Partner you work across all Support at Home pathways, including Restorative Care, End of life and Assistive Technology & Home Modifications, contributing to the mission and strategic goals of Churches of Christ as part of the Seniors Living team.
They provide flexible care planning, education and advocacy to enhance participant safety, health, wellbeing, and quality of life, incorporating wellness, reablement and culturally safe practices. Where clinical needs arise, they partner with qualified clinicians for complex care planning.
As a Care Partner, you will:
Develop individualised and culturally appropriate care plans aligned with assessments, goals, preferences, risks and AT HM needs; ensure care plans identify wellness and reablement approaches to meeting participant goals
