In the role of Transitions Care Lead, you will champion care management while connecting with hospitals and community-based teams. Key Responsibilities:
• Liaise with hospital navigators and support teams
• Review and accept in-home transition referrals
• Prepare initial care plans for timely transitions
• Attend hospital discharge conferences as needed
• Monitor program metrics for quality improvement
Requirements:
