Full-Time Contract - 6 months with possibility for extension | 35 hours per week | Monday to Friday

Working hours: 8:30 AM - 4:30 PM with occasional work outside of business hours. Hybrid position with some travel required within York region for home visits to complete assessment/re-assessment as required.

The Role

Reporting to the Integrated Care Manager, you will be responsible to plan and implement a defined set of community services to support in-home transitions as a core care partner, including supports for Caregivers.

You will be responsible to complete client care needs assessment (independently or with a partner agency), provide care coordination, liaise with relevant CHATS program supervisors and coordinators and serve as the primary contact for CHATS and to provide Caregiver Support and Education within the scope of the program.
