Responsibilities Licensed Clinical Case Manager (CCM)- OTP Clinic The Licensed Clinical Case Manager (LCCM) provides comprehensive clinical case management services focused on diversion and re-entry support for incarcerated individuals with co-occurring mental health and substance use disorders. The role involves assessment, treatment planning, coordination of care, and advocacy to facilitate successful community reintegration while reducing recidivism. The position requires collaboration with correctional facilities, community agencies, and healthcare providers. The Licensed Clinical Case Manager (LCCM) will provide project oversight, clinical assessments, and brief interventions to the participants in the program. The LCCM will also be responsible for holistic comprehensive case planning, record keeping, and providing necessary information to the researcher.
