Case Manager

Provides medical-surgical pediatric and obstetrical case management services to Brockton Hospital patients. Responsible for assuring continuity of care for patients under specific physician groups. Assesses plans, coordinates, and implements plans for quality, cost effective care for patients in the pre-admission, emergency, acute, and ambulatory settings. Is pro-active in the identification of high risk patients for timely case management interventions. There is collaboration with the attending and primary care physicians (PCP's) as well as other hospital clinical and administrative staff to develop and implement effective discharge plans. Monitors the appropriate use of hospital resources and facilitates the delivery of services. Uses accepted criteria and guidelines to conduct review of inpatient admissions.
