Become a Case Manager with LOFT Community Services located in Mount Dennis, Toronto. Help individuals navigate complex mental health and addiction challenges through tailored support plans.
In this role, you'll provide hands-on management to promote personal empowerment and recovery for clients facing mental illnesses and addiction issues. Your expertise will help develop housing solutions and connect clients with relevant community services while fostering a sense of belonging.
Key Responsibilities:
• Develop and implement recovery plans in collaboration with clients
• Assist clients with essential living activities
• Facilitate social supports and community connections
• Execute crisis management plans when necessary
• Organize and promote community engagement events
