Case Manager HIV/AIDS (DOM Infectious Disease) - #Staff

We are seeking a bilingual (Spanish/English) **_Case Manager HIV/AIDS_** who will provide case management to clients with multiple medical and psychosocial needs and across the HIV continuum of care. The Case Manager will coordinate psychosocial support services with community-based social services agencies and on-site substance abuse case managers. They will complete all paperwork, act as an advocate for clients with community-based organizations and public services to reduce barriers and enhance access to care. Will also act as liaison with medical providers and Moore Clinic Social Workers as needed. The Case Manager will participate in relevant training, clinical supervision, conferences, and meetings, and maintain operative knowledge of existing resources for individuals with HIV/AIDS.
