Our services include supports for seniors, caregivers, and people living with mental health and addictions concerns. Role

As a Case Manager, you will work with clients to achieve safe, realistic and reasonable goals within a complex health, social and fiscal environment. The Case Manager's primary function is the provision of quality health care and support services through the effective and efficient use of resources.

What You'll Do

• Initiate intakes and conduct needs assessments;
• Support to individuals with complex mental health, addiction and physical health needs, who have been housed by a Streets to Homes partner agency;
