Job Description

Overview:
We are seeking a compassionate and detail-oriented Bilingual Social Services Coordinator to support older adult clients and their families. This role involves conducting assessments, coordinating services, and helping clients access essential benefits and resources.
Key Responsibilities
Conduct in-home and in-office assessments to evaluate social and emotional needs of older adults and their families
Develop and implement individualized case plans to support client needs
Assist clients with applying for benefits and entitlements within the healthcare system
Identify and escalate crisis situations or cases requiring additional support
Coordinate daily client activities and assist in organizing monthly events
Maintain accurate documentation and support ongoing case management efforts
Qualifications
Bilingual in Russian and English (required)
