Description

Comunilfe Inc is looking for experienced Case Managers for its OMH Adult Homes program in Queens.The Case Manager’s main responsibility is to provide case management services for a caseload of clients. The Case Manager prepares comprehensive assessments and service plans. Home visits and Fieldwork are required.
