A Brief Overview

Delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation in coordination with other health team members. Effectively interacts with the patient, patient family, and other health team members while maintaining high standards of professional nursing. Provides high quality care to the following patient populations: Adult patients experiencing a life limiting illness deemed to be in the last six months of life. Coordinates all hospice services required for optimal end of life care. Manages hospice resources for optimal patient care.The Case Manager works as a part of the interdisciplinary care team, addressing the patient's quality and plan of care, goals and expected outcomes, and readmissions prevention/reduction strategies.
