Center Discharge Specialist

Major activities and essential job functions: Track path results for the rec'd date and providers signature. Task Doctors if paths results need a signature. Calls the lab dept if a path report hasn't been resulted in policy outlined time frame. Review all patient checkouts, enter recalls, task providers team if doctor has ordered any additional test (example of labs, appts, etc.). Close orders (examples recalls, path req, recalls after they have been completed or reviewed, etc.) Assist with calling patients to move up if there are gaps in the schedules. Mail letters to patients if discharge papers are inadvertently left behind. Review PVL for changes to recall order to make sure the recalls were entered correctly after the path's came back. Closes old orders after reviewing for completion. Make patient education copies for nurses (for when they're running low) to attach to discharge paperwork.
