**Job Summary**

To provide high quality healthcare through the provision of diabetes self-management training in a variety of settings to meet the needs of patients, their families and other healthcare professionals. To create and coordinate a multi-disciplinary lifestyle modification program to help participants lower their risk of heart and vascular disease through healthy exercise and eating patterns.

**Minimum Qualifications**

**Education**

+ Bachelor’s degree from an accredited college or university required; master’s degree preferred. Completion of an Academy of Nutrition and Dietetics (AND) approved internship program.
