Provides and/or assists patients with activities of daily living such as grooming, oral hygiene, bath/shower; hair; feeding and nourishment, and incontinence care. Performs and/or assists patients with passive range of motion exercises; ambulation, transports patients or assists patients to walk, transfers to/from bed, floor, wheelchair and shower chairs as needed. Perform various tasks assigned by the nurse including vital signs, height, weight, collecting specimens, food and fluid intake and output measurements Provides accurate, timely documentation; reports care provided and patient observations to RN, including unusual or significant changes in physical, mental, or behavioral conditions. Answer signal lights, bells, or intercom system to determine patients' needs promptly and courteously.
