Chronic Care Management Specialist

Responsibilities:

• Enroll and manage patients in the Chronic Care Management program.
• Conduct monthly phone follow-ups and document patient progress in the EHR.
• Develop and update care plans in coordination with the physician.
• Educate and support patients in managing chronic conditions.
• Ensure compliance with Medicare CCM requirements.

Qualifications:

• Pharm-D, MBBS or any relevant degree.
• Minimum 2+ years of experience in similar role.
• Background as Medical Assistant, or similar healthcare experience.
• Strong communication and organizational skills.
• Familiarity with EHR systems preferred.
• Compassionate and patient-centered approach.
