We’re seeking an experienced Enrolled Nurse to join our national Clinical Care Partnering team, providing virtual clinical support to customers with complex needs and guiding Care Partners through assessment, care planning and escalation.

What you’ll do

· Conduct virtual clinical assessments and case reviews

· Support high-risk and complex customers through proactive monitoring

· Provide clinical advice and coaching to Care Partners

· Review clinical correspondence and support hospital-to-home transitions

· Collaborate with RNs, Allied Health, GPs and service providers
