Clinical Navigation Specialist, Oncology - Cancer Services - IMC

MAJOR FUNCTION:

Oncology Clinical Navigation Specialist will provide individualized assistance to patients, families, and caregivers throughout their Cancer journey from diagnosis through survivorship. The Navigation Specialist will work directly with our Oncology providers and serve as a consistent care coordinator and patient advocate, ensuring timely access and seamless transition across the oncology care continuum. Will be responsible to facilitate rapid scheduling, coordinate care across service lines and sites of care, identify and remove barriers to care, track patients to ensure compliance with treatment and surveillance plan. Provides patient and family education related to disease processes, as well as available internal and external support services and resources.
