 POSITION: Clinical Nurse Specialist Chronic Care - Diabetes Management  DEPARTMENT: Patient Care  CONDITIONS: Full-time
 SHIFTS:  Days7.5 hours per shift (subject to change)  WAGE RANGE:  Scale Minimum MaximumONA04 $41.53/hr $55.69/hr  RESPONSIBILITIES: Clinical Practice Facilitates an interdisciplinary approach to chronic disease management and self-management support as per the Ontario Chronic Disease Management Framework.
 Provides direct, expert, comprehensive care/nursing services, incorporating the Chronic Care Model to improve clinical outcomes for patients with diabetes, through health promotion and preventive care, empowering patients and families to use self-management methodologies to positively affect their health outcomes.
