Clinical Pharmacist

Job Summary
The Clinical Pharmacist plays a critical role in optimizing patient care by ensuring the safe, effective, and evidence-based use of medications. This role works closely with physicians, nurses, and other healthcare professionals to provide medication therapy management, clinical recommendations, and patient education in a hospital or clinical setting.
Job Responsibilities:
Provide expert insights to physicians, health coaches, and nurses regarding drug– drug, drug–nutrient, and drug–lifestyle interactions.
Participate in case reviews, team huddles, and multidisciplinary rounds for complex patient cases.
Review patient medication plans to avoid adverse events, duplication, and contraindications.
Conduct pharmacotherapy assessments, medication reconciliation, and patient education sessions as required.
Counsel patients regarding correct use of prescription drugs, nutraceuticals, supplements, and biologics being used in their Functional Medicine program.
