Clinical Pharmacist

As a Clinical Pharmacist, you will work with our established Clinical Pharmacist Team to develop and deliver medicines optimisation services within our practice. This is an exciting opportunity to join our federation and collaborate with Senior Clinical Pharmacists as part of a multidisciplinary team. Our current team includes 3 Senior Clinical Pharmacists, 12 Clinical Pharmacists, and 7 Pharmacy Technicians. We are seeking a motivated individual to support the continued development of our Pharmacy Team.

This is a patient-facing role requiring clinical assessment and treatment using specialist medicines knowledge across specific disease areas. You may already be an independent prescriber or working towards this qualification. The role involves proactively supporting patients with complex polypharmacy, particularly older adults and those with multiple long-term conditions.
