Community Access Physiotherapist - Adults

We are seeking a Community Access Physiotherapist to join our Program.

Pay Range

$85,000-$95,000 a year, commensurate with experience. Monday to Friday, 8‑hour shift. Full‑time Salary Employee.

Responsibilities

• Provide physiotherapy consultation, assessment, and short‑term intervention to patients transitioning from hospital to home as part of the UHN Transitions Program in the Toronto Central region.
• Complete initial patient visits within 48 hours of hospital discharge and support timely transitions in care.
• Work collaboratively with a Rehabilitation Assistant to deliver care and optimize patient outcomes.
