Job Description
Make a meaningful impact as a Community Care Coordinator at Kenora's Lake of the Woods Hospital. Provide essential assessments and referrals to enhance patient care in a supportive team environment.
This full-time permanent position is ideal for registered nurses, physiotherapists, and other health professionals with at least two years' experience in community health. You will collaborate closely with patients and their families to develop tailored care plans and ensure seamless access to community services through Ontario Health atHome.
