Community Coordinator

Job Purpose: To support the implementation of Community-Based Rehabilitation (CBR) activities by identifying persons with disabilities (PwDs), delivering field-based interventions, facilitating assistive services, and bridging the gap between therapy teams and community needs for inclusive rehabilitation. Key Responsibilities: Conduct CBR-based assessments and assist in developing community-linked rehabilitation plans. Identify individuals in need of therapy, assistive devices, or specialized referrals. Educate and train families and caregivers on disability management and home-based rehabilitation strategies. Facilitate the distribution, demonstration, and follow-up on the effective use of assistive devices and adaptive technologies. Map and maintain an inventory of local resources, services, and facilities accessible to PwDs.
