Community Health Clinical Pharmacist Position

Enhance community health as a Clinical Pharmacist with Vancouver Island Health Authority. Focus on drug therapy problem-solving and medication management across various settings.
In this role, you will collaborate closely with the Seniors, End of Life, and Home & Community Care teams, reporting to the Manager, Pharmacy Clinical Programs. You will provide essential drug counseling and education tailored to client needs while conducting home visits and clinic sessions.
Key Responsibilities:
• Resolve drug therapy challenges for senior and palliative clients
• Provide medication counseling to clients and healthcare teams
• Conduct clinic and home visits as needed
• Optimize prescribing through academic detailing
• Collaborate effectively with healthcare practitioners
Requirements:
• Licensed by or eligible for the College of Pharmacists of BC
• Must have completed an Accredited Pharmacy Residency
• Three years of pharmacy experience in community settings
