Summary:

• We are looking for a non-registered health professional to provide patient support, coordination, and community-based engagement. This role focuses on non-clinical care, including follow-ups, education, and on-the-ground coordination, while working under the guidance of a Registered Nurse or clinical team.

Responsibilities:

• Act as a primary non-clinical point of contact for patients
• Conduct wellness check-ins, follow-ups, and patient engagement activities
• Provide basic health education, lifestyle guidance, and wellness coaching (non diagnostic)
