Overview

As directed by the Medical Health Officer (MHO) of Indigenous Services of Canada (ISC), the Public Health model and concepts of primary health services, the CHN utilizes the nursing process by assessing health needs, epidemiological data, identifying clients needs and establishes a plan of care for individuals, families, and communities in the areas of Communicable Disease Control, STTBI Testing & Follow up, Maternal/Child Health, and School Health.

Responsibilities

• Assess health needs and develop care plans for individuals, families, and communities in the specified program areas.
• Apply nursing process and public health concepts to inform care and interventions.
• Collaborate with team members and stakeholders to address health and social issues affecting First Nations communities.
