Job Title: Community Link Worker – Social Prescribing
Location: Derby (Derby City South PCN)
Salary: £25,750 per annum
Contract Type: Full-Time, Permanent (subject to continued funding)
Hours: 37 hours per week
Working Pattern: Monday to Friday, with occasional evening or weekend work (time off in lieu)
Start Date: ASAP
Driving Requirement: Full UK Driving Licence & Access to Own Vehicle (Essential)

About the Role
We are recruiting a compassionate and motivated Community Link Worker to support individuals referred via GP practices as part of a Social Prescribing service within Derby City South Primary Care Network.
This role focuses on supporting people who may be experiencing social, emotional or practical challenges that impact their health and wellbeing, such as loneliness, mental health difficulties, housing issues, financial hardship or long-term health conditions.
