Location: Community-based – Minehead Medical Centre & patient homes
Hours: Part-time 20 hours available (Full-time 40 hours)

Minehead Medical Centre is seeking an experienced Community Matron to join our primary care team.

If you:

• Are a registered Nurse and a fully-qualified Independent Provider.
• Enjoy building strong, trusting relationships with patients and their families.
• And, desirably, have experience in supporting older adults — including frailty, geriatrics, care homes or complex long-term conditions.
• Can deliver proactive clinical care, polypharmacy review, and anticipatory care planning.
