Job Overview

An opportunity has arisen within the South Kensington Community Health Hub for a triage and assessment mental health practitioner. The role is to screen referrals, undertake specialist assessments and provide brief follow‑up or facilitate alternative service provision for people referred to the team. This is a front‑line post involving face‑to‑face work with service users and their carers. The post holder will need to make clear autonomous decisions based on the complexity of presentations due to the person’s health and social care needs.
