You will work within the Discharge to Assess service in South Nottinghamshire, under the direction of a registered professional and part of a supportive multi‑disciplinary clinical team. The role involves providing community based reablement and rehabilitation support to help patients undertake activities of daily living and remain in their own homes.

Main duties of the job

The Discharge to Assess (Home First) service aims to expedite patient discharge by ensuring efficient and effective reablement care. The team designs recovery and self‑care programmes, performs timely assessments, and collaborates with patients, carers and other professionals. Services operate 7 days a week from 08:00 to 22:00.
