Community Supports Flexible Access and Support Team - Enrollment & Care Coordination Specialist

Description and Responsibilities


The Program works in collaboration with MassHealth to address the immediate daily necessities that arise from the inequities caused by the social determinants of health
The Enrollment and Care Coordination Specialist will screen indivdiuals for CSP eligibility and will complete enrollment for individuals in the program including orientation to services, service agreements, consents, and data entry into the electronic health record software for each member. They also provide training, monitoring, feedback and corrective action to housing specialists and assign and track housing specialists caseloads in collaboartion with the team lead, as needed.
Other Key Responsibilities:

+ Outreach to community providers to develop relationships and develop referral pathways.
